Thank you for choosing ATI College Ultrasound Services. Please read all instructions carefully before booking your appointment.

 

1. ONLINE CONSENT FORM — REQUIRED

All patients must complete the online consent form before their appointment.

2. APPOINTMENT FEE

💵 All ultrasound appointments are $20.00.

Payment is accepted at the time of your visit by Cash or Venmo.
 

3. CHECK-IN LOCATION

DAYTIME APPOINTMENTS — BEFORE 3:00 PM
Please check in with the front desk receptionist on the 4th Floor, Suite 420.

EVENING APPOINTMENTS — AFTER 5:00 PM
Monday–Thursday and select Saturday appointments:
Please check in on the 2nd Floor, Suite 255.
 

4. LATE ARRIVALS

⏰ Please arrive on time.

Appointments may be automatically canceled if you arrive more than 30 minutes late.
 

5. PARKING

🚗 Parking is not validated.

On-site parking is available for $8.00. Please plan accordingly.
 

6. CHILDREN

🚫 Children under 12 years old are not permitted at ultrasound appointments.

Please make childcare arrangements before arriving for your appointment.
 

7. EXAM PREPARATION — IMPORTANT

For OB/GYN, Female Pelvis, and Male Pelvis ultrasound exams:

• Arrive with a FULL BLADDER.

• Do not eat for 8–10 hours before your appointment.

• Drink water only.

• DO NOT EMPTY YOUR BLADDER until instructed by your ultrasound technician.

Following these instructions is important to ensure that we can obtain the necessary images for your examination.
